Cityblock Health to acquire Homeward Health in all-stock deal

Cityblock Health agreed to acquire Homeward Health in an all-stock transaction with financial details not disclosed, the company announced Thursday. Homeward Health provides integrated primary care, behavioral health and chronic disease management services to Medicare Advantage and dual-eligible populations through virtual and mobile care teams focused on rural markets. The combined companies will serve nearly 250,000 people. Cityblock, which currently serves almost 200,000 members primarily in urban Medicaid populations, said the acquisition pairs its data and care-orchestration platform with Homeward's rural provider relationships and care model.
